Impact on diabetes-related health outcomes using MyWay Diabetes platform in Somerset, UK: An interrupted time-series analysis

Abstract

Aims:
MyWay Diabetes (MWD) is a digitally-enabled diabetes self-management tool which provides patient-centred personalised access to electronic health records, structured education, and other self-management features (https://mywaydigitalhealth.co.uk/myway-digital-health-platforms/#_mdmw).We aim to characterise trends in diabetes-related health outcomes (HbA1c, lipids, blood pressure, BMI and weight) for MWD users within NHS Somerset with respect to date of MWD registration.
Methods:
An interrupted time-series analysis in MWD users with type 1 or type 2 diabetes compared pre- and post-MWD registration trends to estimate differences in health outcomes within 48-month pre-MWD registration and 24-month post-MWD registration periods.
Results:
750 (52.3% female, mean age 51.2 (SD15.8)) and 6457 (58.1% male, mean age 64.7 (12.0)) adults with type 1 and type 2 diabetes were included in the analysis, respectively. HbA1c (–8.650 mmol/mol;95%CI: -9.470 to -7.820), triglycerides (-0.120 mmol/L;95%CI: -0.203 to -0.037), systolic blood pressure (-2.520 mmHg;95%CI: -3.750 to -1.290), diastolic blood pressure (-1.980 mmHg;95%CI: -2.780 to -1.190), BMI (-0.256 kg/m2;95%CI: -0.401 to -0.111) and weight (-0.634 kg;95%CI: -1.050 to -0.214) all significantly improved among people with type 2 diabetes at 24 months post-MWD registration. All health outcomes for type 1 diabetes remained unchanged.
Conclusions:
The large HbA1c reduction for type 2 diabetes is unusual for a low-cost scalable digitally-augmented self-management intervention. Future studies should incorporate a controlled study design with a larger type 1 diabetes cohort to mitigate study limitations. This research demonstrates the positive impact of MWD on diabetes-related outcomes and adds to the broader evidence base on digital health interventions for diabetes.
