N2 - People who are receiving palliative care and their families can face constant, complex transitions throughout their journeys. This includes an increasingly complex experience of care dealing with expected decline and managing unexpected crises and changes. Advance care planning (ACP) is one of the strategies that can help people who are receiving palliative care and their families navigate and normalise such complex transitions. Yet, there remain factors that hinder the uptake of ACP worldwide. This chapter demonstrates the use of a card game as a death education tool that facilitates a more comprehensive understanding of the importance of ACP in Thailand amongst a wide range of players: people receiving palliative care, family carers, healthcare professionals, educators and the public. We discuss how the card game provides a facilitated discussion and a psychologically safe space to discuss ACP and how it triggers transitions for the players regarding their ACP. The Multiple and Multi-Dimensional Transitions (MMT) Theory’s lens is then applied and discussed in terms of how the card game leads to subsequent holistic transitions for the players, allowing them to document their future preferences that are reflective of their life goals and values.
