    Abstract

    New adverse effects of aldosterone are now apparent: endothelial dysfunction, myocardial fibrosis and LV remodelling. Aldosterone blockade is able to reverse these adverse effects and so reduce mortality in heart failure. Pharmacological differences between spironolactone and eplerenone are discussed.
